How to book
Referral to Orthotist
Thank you for contacting us to make a referral via our website.
Our commitment to you is to make contact with your patient within 48hrs (weekdays). If they have not received a return call from us within that time. Please ask that they call us direct on the 06 920 3350.
Notes to referrers: Please note when you make a referral we need the following information about your patient:
- Name & Address
- Contact phone number
- DOB
- DOI (if applicable)
- DOS (if applicable)
- ACC claim number (or identify other funder if not ACC)
- NHI number (helpful but not essential)
- Brief description of injury or diagnosis
- Preferred orthotic treatment options.
